Job Description
The Tobacco Treatment Outreach Associate assists the Tobacco Treatment Team Leader and program coordinators in a variety of duties focusing his/her professional skills on potential new patients to enroll, or are considering enrolling, in the in the Smoking Cessation Trust and CIS Commit to Quit Be Tobacco Free Program.

SPECIFIC DUTIES:
The primary duties of this Tobacco Treatment Outreach Associate position are to enhance community presence of the CIS Commit to Quit Be Tobacco Free Program. The associate will utilize his/hers professional skills in the development of community outreach relationships to foster program growth and collaborate with tobacco treatment providers to ensure quality patient care.
Other specific duties include:
1. Assist with management day-to-day promoting of the Tobacco Treatment program on social media.
2. Coordinate with the CIS Marketing Department on all program advertisement and outreach efforts.
3. Complete monthly outreach efforts with potential new patients and various referral resources. Organize and implement community outreach events and presentations.
4. Promote the Tobacco Treatment Program to employer groups and companies seeking onsite cessation services.
5. Assist Team Leader and Houma Outreach Associate with promotion and implementation of Tobacco Treatment Telemedicine services.
6. Collaborate with Team Leader, Houma Outreach Associate and Tobacco Treatment Coordinators to meet outreach needs in all clinic regions.
7. Patient follow up including telephonic contact, program enrollments, assisting with appointment scheduling, coordinating with insurance verification, handling medical records, data collection and entry, etc

TOBACCO TREATMENT PROGRAM:
1. Complete outreach and marketing efforts to promote CIS Commit to Quit Program. It is expected to have a minimum of 2 outreach events per month outside of regular business hours to meet the community demands. It is expected to have a minimum of 2 direct service outreach events per month.
2. Develop presentations for Employer Groups and organize meetings for Team Leader and Coordinators to best represent the company in the community and professional systems.
3. Aid in scheduling Tobacco Treatment medical appointments and counseling services for CIS patients.
4. Provide individual counseling sessions weekly per volume expectations either telephonically or face to face. Location, days and times for the counseling sessions will be determined by the needs of the patient(s).
5. Assist Tobacco Treatment Coordinator with providing counseling sessions in a group setting and telemedicine services as needed with a possible minimum of two days a week at various CIS clinics or off-site locations. Locations, days, and times for the group counseling sessions will be determined by the needs of the patient(s).
6. Serve as a resource for the CIS Commit to Quit Be Tobacco Free Program.
7. Compile and provide statistical presentations demonstrating counseling visits by a provider, group, and individual counseling.

DOCUMENTATION/RECORDS/BILLINGS
1. Complete weekly and monthly outreach summary report.
2. Verify the Eligibility of Benefits for those patients utilizing smoking and tobacco use cessation benefits provided by the Smoking Cessation Trust.
3. Document subjective, objective, and assessed clinical findings in the counseling progress notes so that all members of the health care team may treat patients holistically.
4. Complete all necessary and required documentation for each patient and outreach encounter.
5. Provide/Submit pertinent information to program assistant, clerical personnel and Team Leader concerning insurance verification and billing for services rendered.
